633 FSS/ID Card office is transitioning the ID Card appointment scheduling from Setmore to DMDC on 1 April 2024. All available appointments in Setmore are booked between now and 31 March 2024. While the team transitions to the new DMDC appointment system (estimated go-live 4 March 2024), the ID Card office will accept both walk-ins and phone calls to schedule appointments Monday through Friday (07:00 – 15:30, except Tuesdays 07:00 – 1400), commercial # 757.764.2270 or 757.764.3010, Option 1.

For your awareness the new appointment system will be located on the Official DMDC ID Card Website, search your applicable zip code, and select location “JBLE, Langley AFB” for ID Card & DEERS appointments at Langley AFB (https://idco.dmdc.osd.mil/idco/).

Military Personnel Flight

The MPF is the focal point for Air Force Personnel matters! Located in the 633rd Mission Support Group Building at 45 Nealy Ave (Building 15), we can assist active duty members, their dependents, Guard/Reserve members, retirees, and DOD Civilians with a variety of services including CAC cards, passports, and more.

Uniformed Service Members
Members can request a Military CAC while in civilian attire, however, in order to take a military picture the member must be within grooming standards. If you are on a shaving waiver, ensure you have it with you to present to our office.

All Personnel Requesting a Military ID
Ages 18 – UP must have 2 forms of ID (1 unexpired official picture + 1 official picture/non-picture) that bear the name reflected in DEERS.

Ages 10-17 require (1) official form of Identification Example: Birth Cert or SSN Card

Eligibility Documentation
All eligibility documentation must be original or certified copies. A certified copy is a copy of a document or record signed and certified as a true copy by the officer to whose custody the original is entrusted.

Sponsor Responsibilities
Sponsors must be present for any/all DEERS transactions.
If the sponsor is unable to be present, an original unexpired Power of Attorney (POA) must be used in lieu of the sponsor or an original DD FORM 1172-2 (ID card application) signed by the Sponsor (within 90 days) and certified by a verifying official.

Sponsors have the responsibility to report any family member additions or removal changes to DEERS (Defense Eligibility Enrollment and Reporting System) within 30 days of event.

Initial Enrollment
Below is the required information for initial enrollments for the following:

Non-Military Spouse:
Marriage License (Original or certified copy)
Birth Certificate (Original or certified copy)
Social Security Card (with desired name)
Valid photo ID (with desired name)

Child:
If the child is over 90 days old, the birth certificate and social security card will be required to enroll the child.

Stepchild:
If a stepchild is being added, the marriage certificate is also needed.

*Additional documents may be required based on certain circumstances

Child born to unmarried male sponsor:
Voluntary acknowledgement of paternity, birth certificate and SSN card are required for DEERS enrollment

Full Time College Students Age 21-23
Must bring a letter from their schools registrar’s office (official school letterhead required) stating the full-time enrollment status & the course of study leads to the award of an Assoc. degree or higher. Estimated graduation date must be included.

The student must be enrolled full time in an Accredited School in pursuit of an Assoc. Degree or higher. Refer to AFI36-3026_IPV1 4 Aug 2017, Table 4.4.for additional information.

*NOTE: A Student ID CAN NO LONGER be used as a DoD approved form of identification.

Incapacitated Adult Child Age 21 and Up

Legal Custody Ward

When to Turn in ID Card
Please bring in your CAC card for destruction once it expires. Common expiration periods include:

– Permanently Incapacitated: No expiration date
– Retired: No expiration date
– Divorce (Civilian Dependents): ID Card expires 1 year from date of issue or on date divorce is final
– Spouse: ID Card expires on the end date of member’s active duty orders or date enlistment contract expires
– Child (Unmarried under 21 years old): ID Card expires the day before 21st birthday, end date of member’s active duty orders, or date enlistment contract expires
– Child (Unmarried over 21 years old): ID Card expires the end date of member’s active duty orders, date enlistment contract expires, expected date of graduation, or day before 23rd birthday
